I tried an app but I never kept up with it. Now I use a pretty basic form that says date, migraine or headache, medication taken

I recently started tracking my migraines again in preparation for seeking another neurologist. I came across a fantastic app for tracking - Migraine Buddy. Very detailed. Android users can get it using Play Store.

I've tried using Migraine Buddy. I actually really liked it. Now I use a log my neurologist gave to me.

I use Migraine Buddy App on the phone, I used to keep a book but I was terrible for remembering to fill it in when I was feeling better, the app reminds me and allows me to record, meds, reliefs what works and doesn't and also symptoms and potential triggers, the app also creates reports over time to help you build up patterns and triggers that you can export for doctors etc...
