I am a retired pediatric nurse practitioner. A portion of my career was in Child Neurology at a large teaching hospital, where we saw and treated adolescents with migraines. We used Amitriptyline effectively in managing migraines in adolescents, especially those with aura. With many of those we treated there was a degree of anxiety as well as, in relation to, having migraines. Amitriptyline managed both the migraine and the anxiety.

It never worked for me, but our migraines are probably different.

Silent migraines (migraines without head pain) can be tricky to treat, and there are currently no FDA-approved treatments specifically for them. However, preventive medications used for migraines with aura may help.

Amitriptyline is a tricyclic antidepressant (TCA) that is commonly used as a preventive migraine treatment — meaning it's taken daily to reduce how often attacks happen, not to stop one once it starts. It works by increasing serotonin and norepinephrine in the brain, which can reduce nerve sensitivity. One MyMigraineTeam member shared: *"I was on a tricyclic antidepressant for 20 years. I got a fast heart rate, dry mouth, and delayed stomach emptying. I slowly got off the drug but couldn't find any other drug that helps my migraine as much."*

Common side effects of amitriptyline include:
- Dry mouth
- Drowsiness
- Constipation
- Blurry vision
- Weight gain

Because it can be sedating, it's usually taken at night. It may take up to four weeks to reach its full effectiveness. It's always best to speak with a healthcare provider to see if amitriptyline is the right fit for managing silent migraines specifically, as treatment plans can vary greatly from person to person.
